To avoid any problem in the search process, it is ideal to provide the basic information of the record being requested. The personal information of the one requesting for the document has to be indicated on the request form as well. In case where it is not possible to go to the office to file the request, one can request it by mail. However, the requirements and needed information must be complete and enclosed to the mail request. A maximum of 10 working days is needed for the search results to be sent back.
